STI Testing

Counseling
Provides individual and group support and counseling.

Emergency Financial Assistance
Assists clients with essential utilities and basic telephone, critical need items, emergency medications and short-term housing assistance.

Food Bank
Provides food, food vouchers and personal care items to help with taking medications.

Health Insurance Assistance
Provides assistance with insurance premiums, co-pays and deductibles for medications, outpatient primary medical care, dental and vision for clients.

Home Delivered Meals
Provides readymade meals to clients who are homebound.

Non-Medical Case Management
Provides advice and assistance to empower clients to access a variety of necessary services (including medical, social, community, legal and financial).

Services Provided

Housing Assistance
Provides rental assistance up to 12 months per client in a grant year, excluding emergency/transitional housing assistance providers.

Legal
Assists clients with legal matters such as documents relating to personal and estate planning, discriminations, notarial services, debtor problems or apply for public benefits (SSI, unemployment).

Medical Case Management
Assists PLWH to navigate the health care system, to enter and remain in care, assist with application for services such as Medicaid, Medicare, ADAP, food, housing..etc.

Food Bank
Provides food, food vouchers and personal care items to help with taking medications.

Medical Transportation
Assists with transportation to medically related appointments using resources such as public transit tokens and/or passes, gasoline vouchers, taxi services, or rideshares for appointments.

Medications
Provides HIV related medications to individuals with no insurance or low coverage insurance.

Primary Care
Provides primary care visits with doctors or nurses and labs related to HIV free of charge.
